Q: Where do I go to sign my student up for their flu clinic online?
A: Please follow the following steps:
1. Use this link to the VINI website: vini.nh.gov/providers/s/get-flu-vaccine
2. Use the clinic code from your school’s announcement letter AND your school’s zip code to     search for the clinic
3. Enter your child’s information and follow the prompts on the screen
4. When finished, you will be brought to a confirmation screen with your appointment information and a QR code, and you will receive a confirmation email (check your spam folder!). If you are not brought to a confirmation page, your registration was not successful and you should try again, or contact your school nurse for a paper form.

Q: I can’t find my school’s clinic, what should I do?
A: Please search using the school code included in your school’s announcement letter AND your school’s zip code. If you do not have the letter, contact your school nurse for the information.

Q: Does my student need to bring proof of residency or their QR code to the clinic?
A: No. VINI is a new website and we are still working some things out – your child does not need proof of residency to attend the clinic, and we are not using the QR codes at this time.

Q: What is the NHIIS and why is my child’s information being entered there?
A: New Hampshire’s Immunization Information System (NHIIS) is a secure, online platform that brings together information from different health care providers into a single permanent record. Your child’s immunization record is available to you at any time. Additional information is available at dhhs.nh.gov/dphs/immunization/nhiis-public.htm.

Q: How do I sign my student up if I don’t want their information logged into NHIIS?
A: If you would like your student to be vaccinated, but do not want their information logged into the NHIIS, please contact your school nurse for a paper consent form. YOU MUST ALSO COMPLETE THE OPT-OUT FORM that can be found using this link: dhhs.nh.gov/dphs/immunization/documents/nhiis-opt-out.pdf Please complete this form and keep it with your student’s paper consent form. They should bring both to the clinic
Please note: any paper forms that do not have the opt-out form with it will be logged into the NHIIS – this information can be removed upon request by contacting SCPHN if opt-out form was lost or if you change your mind.
