Community Health Improvement Plan (CHIP) 2018-2021

The Strafford County Community Health Improvement Plan (CHIP) is an action-oriented strategic plan and framework to be used to leverage resources and to engage and mobilize community stakeholder organizations to address barriers and opportunities to improve community health.

The CHIP is our County-wide plan to help improve the health of our communities. We do this by pulling together organizations and community members to help improve programs and services that help keep people well. Our new CHIP will focus on:

      1. Mental Health
      2. Substance Use Prevention, Treatment, Recovery & Harm Reduction
      3. Healthy Living, with a focus on Food Security & Chronic Disease Prevention
      4. Emergency Preparedness
      5. Access to Healthcare Resources including Oral Health, Prenatal & resources to prevent Sexually Transmitted Infections (STI's)
      6. and Injury Prevention, including Dating/Domestic Violence, Lead Poisoning, and Suicide Prevention.
