The Strafford County Community Health Improvement Plan (CHIP) is an action-oriented strategic plan and framework to be used to leverage resources and to engage and mobilize community stakeholder organizations to address barriers and opportunities to improve community health.
The Community Health Improvement plan (CHIP) is your county’s plan to help improve the health of your community. We do this by pulling together all of the organizations and people that have an effect on the programs and services in your community that help keep you well. Our current CHIP plan includes a focus on
- Healthy eating and exercising
- Heart disease and stroke
- Mental Health
- Drugs and alcohol
- Keeping the public safe during emergencies and natural disasters
