DESCRIPTION:NH Hunger Solutions is excited to announce the roll out of NH Care Connections\, a partnership with Unite Us\, closed-loop referral system\, and the Department of Health and Human Services. This program will help us connect Granite Staters to the services they need to improve their access to food\, housing\, education\, child care\, and other assistance programs. It will also act as a digital resource guide! \nPlease join us in person on November 19th at 3:30pm at Frisbie Memorial Hospital Conference Center\, 11 Whitehall Road Rochester\, NH 03867 for the official NH Care Connections Kick-Off Training. Light refreshments will be available. NH Care Connections is a closed-loop referral network that includes providers that offer services such as healthcare\, housing\, employment\, food assistance\, mental health\, utilities\, education\, veteran services\, and more. NH Care Connections is being rolled out to all health care and human services providers and community organizations\, including health care practices\, managed care organizations\, educational providers\, Rapid Response Access Points\, Aging and Disability Resource Centers\, the NH Doorways network\, 211 call center\, and others. \nThe goals of the network include improving people’s experiences navigating the healthcare system and improving health outcomes. NH Care Connections ensures providers can quickly communicate with one another and their patients to coordinate care and ensure people have positive health outcomes.
